What is a Family Health Team (FHT)?
  

  A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient.
Designed to give doctors support from other complementary professionals, most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible.
Do you know that you can request to see a Family Health Team Nurse Practitioner, Nurse, Social Worker, Dietician or your Pharmacist for many of your medical problems?  As well, you can request to see the Diabetes Nurse Educator or go to the Northwestern Health Unit instead of your doctor for certain problems.  By accessing other professionals of the Points North Family health team, you not only receive specialized care, you may be seen more quickly and by the same health care provider each time you visit for the same issue.  
As a member of the FHT, each professional can consult with a doctor if needed.  Look at the chart below to see the areas of expertise each care provider can offer you.
